Cagrilintide is an investigational, long-acting metabolic peptide that represents the next major frontier in weight management and type 2 diabetes treatment. Developed by Novo Nordisk, it is a dual amylin and calcitonin receptor agonist (DACRA).  

While current heavy-hitters like semaglutide (Ozempic/Wegovy) and tirzepatide (Mounjaro/Zepbound) rely on mimicking gut hormones like GLP-1 and GIP, cagrilintide targets a completely different physiological pathway: the amylin system.  

How It Works in the Body

Amylin is a hormone naturally co-secreted with insulin by your pancreas when you eat. It tells your brain you are full, slows down how fast your stomach empties, and suppresses glucagon (a hormone that raises blood sugar).  

Because natural amylin degrades in minutes and is highly unstable, cagrilintide was engineered with structural modifications (lipidation) so it can be injected just once a week.

The "CagriSema" Super-Combination

By hitting both the GLP-1 receptors and the amylin receptors simultaneously, the combination attacks appetite from two completely distinct angles.  

Latest Clinical Data: In the phase 3 REIMAGINE and REDEFINE clinical trials, CagriSema demonstrated massive efficacy. For patients with type 2 diabetes, the high-dose combo lowered HbA1c by up to 1.91 percentage points and drove an average 14.2% body weight reduction—significantly outperforming semaglutide alone. In pure obesity trials, patients achieved an average weight loss of 22.7% over 68 weeks.  

Side Effects & Status

The safety profile of cagrilintide is highly similar to the GLP-1 class. The most common side effects are gastrointestinal—including nausea, diarrhea, and vomiting—which are typically mild-to-moderate and taper off over time.  

CagriSema was submitted to the US FDA for weight management approvals, positioning it as a major upcoming player in metabolic medicine.
